The Pain fruit isn't used very often, which makes it a bit underrated. It's not good for defeating lots of monsters quickly (grinding) and doesn't protect you from certain elemental attacks (lack of Elemental reflex).
However, it can be useful in fights against other players (PvP) because it has attacks that reach far and lets you move around decently during battles, especially with its Self Repel move. This move helps you push away enemy attacks and stun them, which is great for PvP. But, it takes a lot of skill to use effectively.
The Pain fruit isn't worth much in trades. It's often used as a filler item, along with the Gravity fruit, because it has almost no value.
